Dental Implant 10-Year Warranty - Resource 1
As an active member of the Resource 1 Dental Implant Benefit Program, your dental implant treatment is protected under our 10-year replacement warranty program.
HERE IS HOW THE 10-YEAR WARRANTY BENEFITS YOU:
ACDI IMPLANT NETWORK PROVIDER:
Once treatment has been completed by an ACDI Implant Network Provider, all registered manufactured dental implant components used in your treatment are under warranty for ten years from the date of placement. Should alteration or replacement of the implant components become necessary, Bio-Life Implant Systems will replace the needed components at no charge to the ACDI Network Dentist, and you will not be charged for the new dental implant components.
NOTE: Implant prosthetics (crowns, bridges and dentures) are not included in the warranty)
WARRANTY SERVICE AREA:
The Resource 1 Ten Year Warranty travels with you anywhere in the United States where there is an ACDI Dental Implant Practice. If you are away from your original network provider, your treatment is still under warranty in another city or state. Simply call Resource 1 to find an ACDI Network Provider close to you and present your Resource 1 membership card there. Your treatment benefits still apply. (Schedule of treatment benefits vary from state to state.)
SERVICE FEE:
If you should need corrective implant treatment within ten years of your original surgery date, you can be charged a service fee by the ACDI Network Provider, for the implant replacement treatment. This fee can be between 30-50% of the Original Scheduled Fees for treatment found in your Resource 1 Fee Schedule. Fees are subject to change throughout the ten years of your warranty.

CONTINUATION OF COVERAGE:
The ten year warranty will remain in effect as long as the member:
1. Remains an active member of the Resource 1 program through membership renewal policies
2. Receives at least two (2) professional maintenance visits per year from your ACDI Dentist (Proof of maintenance cleanings will be required by Resource 1 upon replacement or alteration treatment)
